BILLING
We are contracted with PacificSource Commercial (not OHP or Medicare), Regence BCBS, Providence Commercial, Moda, Aetna and First Choice Health Network. As this is a small practice, you are responsible for being aware of your coverage and deductible.
We offer a same day cash pay discount for patients that have insurance we are not contracted with or those that prefer to pay privately. Payment accepted includes cash, check, Venmo, HSA/FSA, and credit card.
SCHEDULING
Step 1: Reach out via text, phone or email, or schedule an appointment with me here, we offer both in person and virtual appointments.
Step 2: Once you have a confirmed date and time, you will receive an email from Charm, our electronic health record system, with consent forms to sign and an intake form to complete before our first visit. Please fill this out in advance as it helps me prepare for our visit.
Step 3: You will have your visit with me. Initial visits are 50 min and follow up visits are 30-50 min. I will listen to your concerns and together we will form a plan, including any lab work or medications that may be indicated.
CANCELLATION POLICY
Your sessions are scheduled in advance and we reserve that time exclusively for you. If you must cancel or reschedule, for any reason, please notify us by phone/text or email a minimum of 48 hours in advance. When you cancel or miss a session without the required notification we are unable to fill the time slot for another person and you will be charged a $100 fee.
CAN YOU BE MY PRIMARY CARE PROVIDER?
No, we do speciality medical care and thus cannot serve as your PCP. We may be able to help with some primary care concerns but it is necessary to have a local PCP. Emily has been in this community for a long time and has many wonderful recommendations.
